A quick search of "event lighting failure" doesn't really reveal much. Outside of the obvious complete power failure Event lighting can "fail" and when it does its not fun. it can "fail" from lack of acknowledge,actual equipment failure,or sometimes just inattention to detail...
Proper equipment is the first place to start. Make sure you are using the right equipment for the right job. Surprisingly this can be more complex than just proper size, & Quantity. Are you using a "flood" light when you really need a "spot" light? When it comes to power, Understanding what power is available to you & the needs of the entire event it critical (is their any energy hogs such as a coffee pot?) Having to reset the breakers can Quickly ruin the mood of the event.
Think Safety. The possibility of tripping and running into things can increase with event lighting. Secure all cabling & Ensure there is enough lighting by stairs, exits, cords, light poles etc. In case of emergency make sure you know how the controls work in case you need to turn them all on quickly.
Regardless of whos doing the event lighting, by keep the above in mind the lighting will be successful
